Each radio is programmed with such a code. You'll need to pull the radio - there will be number on a label on the top (IIRC) - take the car and this number to a dealer and they can research the code. You will likely need to prove that you own the car before they'll give it to you. -Justin P.S. Holding the BAND button does not avoid having to enter the code... if you enter the code incorrectly three times in a row, the radio will lock and stop asking for the code... holding the BAND button (and some other stuff, IIRC) will reset the radio to the COdE prompt...
