Here a few more ideas: I improperly installed a timing chain once (off a tooth) and the car ran fine, but made a terrible tappet noise. It is easy to check this by lining up TDC on the flywheel and looking for alignment of marks on each cam shaft with a mark on the cam bearing mount at the rear. Look at a service manual for the procedure. Also, the guide in the top of the valve cover can get hard and make a rattling noise as it is slapped by the chain.
