An intermittent injector problem like that is possible, but doesn't seem likely. My 1991 9000T has the same setup as yours (DI and Bosch FI), and has over 130000 miles on it (about 210000 km). I noticed the fuel economy improved quite a bit - about 3 miles per gallon - after I cleaned the injectors. Have you tried replacing or switching the spark plugs? Examining the plug that isn't firing might give an indication if there is fuel but no spark, or if it is spark without fuel. Maybe the electrical contact to the spark plug in the DI cassette isn't making good contact. Or it may even be a faulty spark plug. Good luck!
