How, exactly, did you check the fan? If you put 12V on the input wires and it spins, it could be that the output transistor is blown or that the control unit is bad. My bets are on the transistor. http://www.quasimotors.com/transistor.htm If this is blown, there is probably a cause other than old age. If the motor armature is worn, or its bearings, it will draw excessive current and kill this transistor. If there is insulation or water on the fan basket, it can also be a cause. If it is the original fan motor, it probably needs replaced, anyway. If you go that far, replace the heater core as well if it has not been replaced.
