It was common on the old 900 auto box because instead of having two notches cut into the snout, it was two drive tabs sticking out making the whole snout more vulnerable especially when oil pump bearings got worn and the whole oil pump gears started to wobble around in there.
It is also easy to damage the torque converter if it is not seated all the way into the gearbox by hand before mounting the box to engine.
When you tighten the bolts between engine and transmission you damage the splines on torque converter or oil pump.
