Sounds like maybe the auto-adjust mechanism inside the caliper isn't keeping up. As the pads wear, the hydraulics push the piston a bit further & further. The mechanical bits that are operated by the cable are supposed to take up that slack. If that's the case, maybe a new or rebuilt caliper is the answer. I don't know whats involved in rebuilding that yourself.
Done any work to the rear brakes lately? If so, you know about twisting the piston to get it retracted. Possible that you retracted it "too far"? I think that would result in the brake working OK by the hydraulic system but not by the cable.
