Shifter Alignment

I've got a 98 9000 with a manual transmission and have been struggling to align the transmission after installing a short shifter. I've followed the procedures using two screw drivers to lock the transmission and shifter in 4th gear. Have loosened the pinch bolt and retightened. At one point I didn't have first or second gear now I've lost reverse too. Any suggestions? What am I missing. The shift coupler is the next style with a little wear on the bottom side.
It was shifting fine with the old shifter just was worn out and would loosen overtime (pull up) and the spring would pop off.
