PAX grinding noise

Recently replaced passenger axle (torn boot) and then drove to Socal and back (about 2000 miles). then had a wheel bearing noise, so I replaced the hub. At the time I replaced the hub, I realized I didn't secure the inner CV boot well enough, so it threw all the grease out. I regreased the CV and all was quiet for about 300mi.
Now the wheel bearing-like noise is back. However, it doesn't have the load/unloading sound when going through corners.
Also, after starting the car cold, when I turn the wheels to pull away from the curb, there's an awful grinding noise. Disappears after 50yds and I won't hear it again until I start from a cold stop after several hours parked.
The brake caliper looks seated, there doesn't appear to be any disc or pad contact in the wrong places. There's no additional dust or buildup on the wheel.
When I did the hub, I had to reuse the axle nut and I'm not positive I got it back on to the 280ft/lb (I thikn it's that high) since we didn't have a torque wrench that went that high.
Is this a worn axle due to driving 2000 miles with low/no grease or failed wheel bearing?
