I have meniscal tears in both knees, and about 2 years ago, the orthopedist was ready to do the surgical repairs, except that I wasn't allowed to come off the Plavix for a recent cardiac stent.
(Interestingly, it was the walking on the treadmill as part of the cardiac rehab regimen that I was in that really aggravated the knees, prompting the orthopedic visit).
Once I could come off the Plavix, I went back to the orthopedic surgeon figuring we'd be scheduling surgery. Instead he decided to try PT. I did about 8 weeks of 3 days a week PT plus the stretches etc. the PT prescribed, and its worked really well.
I too wear the cushioned insoles (Dr. Scholls brand that you get in CVS after the kiosk evaluates your feet) and for work / commute a pair of black Reebok shoes (like referee shoes). Using these as consistently as possible has really helped.
My commute has recently moved to a 2 to 3 day a week public transit commute (versus my beloved Saab), which has roughly trebled my walking on those days (up to about 7,500 steps), including up and down subway stairs, sidewalks and some cobblestone. My knees generally feel good, although icing them down at the end of the day feels very nice as well.
