People have suggested a faulty temperature sensor and similar things, but the behaviour of my ACC makes me doubt these: it's trying to do the right thing, kicking in at the right time, etc, it just only blows warm air. If you've actually tested these things and found them to be working, that seems to confirm my thoughts. It's a mystery, and very annoying on long journeys when it's raining!
