The heater core always gets flow from the engine - it's hot all the time. When you adjust the heat, what you're really doing is adjusting a 'mixing flap'. This is just what it says, a plastic flap that mixes hot air from over the heater core with cold air from outside/AC. More heat - move the flap to pull more air from the heater core. You get a much faster heat/cool response than by adjusting the flow of coolant through the core, and less leakage. Most cars use the flap method.

The flap is mounted in the plastic box with the heater core. The flap is on a shaft, and the shaft comes out of the box. An arm on the shaft connects to the heater control (knob for manual, servo for ACC).

Sometimes the plastic arm breaks off, and the flap falls to heat (if it's summer), or cold (if it's winter), and can't be adjusted. There is a fix kit for it that can be installed quickly and easily. But if the arm breaks off right at the shaft, the kit won't work, and the entire flap must be replaced. By the time you've gotten to the ($25) flap, you've got the heater core out.

There is no maintenance for the flap, and if the arm breaks, it usually breaks such that the kit (about $8) works just fine.
