It should be done every 30K miles (at least) together with the major service.
It is not included in the regular service schedule outlined in your service book and some shops charge an hour to do it, some 1/2 hour.
I do it for free (on most cars)when completing the full 30, 60, 90 K services.
Some older Saabs and Volvos take a lot of extra work to clean out throttle bodies and adjust idle system.
What a good mechanic also do at the same time is to check over the crank case ventilation system (PCV) to make sure all hoses are in good condition and that the PCV check valve is working properly (that is outlined in your service book though).
You can search for the topic on the other boards since it might not have been talked about that much on this, but the procedures are about the same on most cars.
