D37 -- I've had the same problem recently with my manual '04 9-3SS 2.0t.
The shift knob popped off twice in two days this summer when I was driving in light city traffic -- fortunately I was able to pull over and quickly jam the knob back into position, although I wasn't able to center it as precisely as I would have liked and it is now a bit off center. It hasn't happened again since, although I am considering taking it to the dealer to have them re-center and secure it altogether.
Like you, I didn't notice any kind of break; it came off cleanly. I have a feeling that it was due to the extreme heat in my area; I live in Washington DC and park my car outside while at the office, where the interior is routinely subjected to 90-100+ temperatures. It's possible that the heat, combined with my overly strong grip on the shifter, that caused the knob to pop off.
Other than that, I've had no problems with the car.
