I've never replaced one either, a few 5-speed autos in the 4-cylinder cars, but that was all for water intrusion from broken radiator.
Have yet to have a V6 engine fail either, though I have talked to a tech or two on the East Coast who have seen one or two. I've also never had a 6-speed manual go, apparently 4th likes to diverge from the other gears on those sometimes.
Only 4-cylinder motor we had to replace was because it still had the original spark plugs at ~155k miles, the owner put a tuned ECU in it, and it cooked the electrodes off all 4 plugs, and chewed them up in the chambers, messed up all the pistons and most of the valves. Wasn't pretty.
One of our mechanics ended up with that car.
