With proper, regular oil changes, and keeping an adequate oil level, the timing chain is good for the life of the car, well over 200K. Yes, occasionally some fail sooner, but that's rare.
There are a lot of things on the belt end of the engine that can make a rattling noise in addition to the chain. Unless it is VERY loud and rattly, I don't believe that anyone can diagnose the timing chain by ear.
On my '88 9000T, I had a tech tell me that the timing chain was about to go, 'by ear.' At around 90K I pulled the valve cover to replace the gasket, and my inspection of the chain showed it to be fine. And just as noisy. It was still just as noisy at 140K when I got rid of the car.
Remember, there is no downside for the mechanic. Should lightning strike and the timing chain break, he's covered. If it keeps you up nights and you bring the car in to having the timing chain replaced, the mechanic just got a $1000 job. If he opens it up and finds the chain in great condition, do you think he'll button the car back up and not charge you?
I don't dread the timing chain. They are quite robust. Change the oil, and keep the level up near max, and the chain will outlast the rest of the car.
