This has happened to me before. My cassette adapter only works on one side (I can't remember if it is A or B). If the radio is set to play the wrong side when I insert the adapter it will do what you described. If I push the button on the radio to flip sides (before it gives me the error message) it works fine. The audio source must be playing when I insert the adapter.
