If the belt is hanging "off the lip" on the tensioner pulley, you need a new tensioner assembly.
The nylon disc between the outer and inner part of tensioner is most likely worn off.
That disc works as a sliding cushion between the fixed and moving part of tensioner. and when it is worn out, the tensioner pulley tilts slightly and the belt start to wander off.
