I have always replaced the serpentine belt at 60,000 mile intervals as this is an item in the 60K Major Maintenance Interval Procedure.
At 105,000 miles I replaced all three pulleys. The small idler pulley had a distinct clicking sound when I spun it on my fingers after removing it. The tensioner puley and center idler pulley sounded fine.
At 180,000 miles I replaced the small idler pulley a second time when I replaced the belt. Again, with 75K on this pulley there was a clicking sound from the pulley bearing, but very faint compared to the sound previously with 105K on the bearing.
I plan on replacing all three pulleys when I hit the 240,000 mile point.
In summary - 60K for the belt and small pulley; 120K for the tensioner and center idler pulley.
All these belt and pulley changes were D-I-Y'ed.
