I rotate about every 5k. front to back. I have unidirectional tires. Rotation and balance are free at my tire store.
at about 12k on the tires, I was getting road noise. Looking at the tires I could see that the rear inside was wearing a bit faster and I could see the early signs of cupping. Rotating them to the front helped a bit but by 15k the noise was loud.
At that point I had the tires flipped on the rim, "crossed" and rebalanced. It cost me $20 total. The noise was gone, and the tires are wearing much better now. I am at ~25k on the tires now and they are nearing the end of their natural life. They are worn very evenly.
I believe that if I had not flipped them they would not have made it past 18k.
YMMV
