Using a good flashlight, see if you can see the piece. I personally don't think there is much danger in turning the engine over by hand unless this piece is so small that it could become wedged between the piston and the cylinder wall. But even if you move the piston to the top of its travel, you've still got to get in there and retrieve the piece. There is a claw-type tool that you can buy at many auto parts stores. It is about 2 1/2 feet long cable (sort of like a brake cable on a bicycle) with a four-prong claw on one end and a plunger on the other end to control the claw. Push the plunger and the claw opens/expands, and release the plunger and the claw grasps whatever you're trying to retrieve. The claw end is fairly small and will likely fit through a spark plug hole. I've used it several times to retrieve tools, nuts, bolts, etc. from the most remote places. Failing that, find a piece of rubber hose small enough to fit through the hole and secure it to the hose of your vacuum hose with tape, and use that as your fishing pole.
