Just throwing ideas at the wall. I might consider the following
1. Could the supply line be plugged or kinked enough prevent sufficient fuel flow?
2. A friend's truck had this type of problem when his EGR valve plugged up. Could there be a problem with exhaust gas recirculation?
3. Did the CPS fail?
4. Are you having a throttle body problem?
5. Did limp home get set?
6. Is the catalytic converter plugged?
7. Did you get some bad gas. is there water sitting in the bottom of your tank that's being drawn into the lines?
8. Do you have access to a spare DIC to try on one bank, then the other? Lacking that, clear the code, swap DICs from bank one to bank two and see if the code changes to identify the new bank.
That'a some thoughts that came to mind. Really do hope you get it solved soon. Please post with what you eventually find out that it is.
