I've had the floor mat thing happen to me ('89 Ford Probe GT - nice, fast car). Smacked the brake, popped it in neutral, switched off the ignition as engine was overrevving, coasted to the side of the road, straightened things out and proceeded on my way.
This report was related to the 'unintended acceleration' scares where people are stomping the gas pedal and think they are on the brake.
A Lexus ES350 is a vehicle that (let's say) would not typically be driven by a knowledgeable car enthusiast, and may have a higher number of these types of occurrences.
