Very strange inner brake rotor / pad wear

I did my front pads the other day. Replacing pads on 60K old rotors which had Akebono ceramics on them from mile one. The outer rotors and pads are smooth as glass, no grooves no scoring, pad surface straight and even, just perfect gentle wear! The pads themselves had life left but I had a box burning a hole in a basement shelf and just wanted them on.
However, the inner pads have this very strange wear: The outermost 1/6th of the inner rotor surface is raised (perhaps warped?) and the topmost 1/6th of the corresponding inner pad is more worn with clearly visible grooves in it while the rest is gently worn and smooth as glass.
It's almost as if that portion of the inner rotor didn't have braking surface on it since that portion (on the rotor) is raised and on the pad (less pad material, more worn) could something be caked on? I didn't get a good look.
Should I have the inner surface turned? I have vibrations under hard braking on highway, was thinking of just grinding through it for a while...
THOUGHTS?!??!!?
