serpentine belt damage - need advice

I replaced today second failed serpentine belt on our 2000 LPT wagon (100K miles) needed in the past 6 months. A new water pump was installed with new belt at a dealer. That belt failed ~3 months ago with internal edge fried and fractured. The replacement belt and new idler pulley were installed. There was also alternator failure and replacement within this time - work done by indie.
The belt failed yesterday again with inner edge stripped around half it's length. When installing this one myself, I observed that it tends to ride over water pump too close to timing belt cover and even rubs into it cutting a groove. I retracted the tensioner and pushed the belt to edge of the smooth pulley but it returns to the inner edge after few revolutions, tried few times.
It also likes to ride close to outer edge of the smooth idler pulley, in my opinion. On other pulleys grooves are engaged on the inside.
Is there a way to align all accessories to compensate for offsets and force the belt to run away from timing cover? I suspect that rubbing causes the damage.
Would a SAAB water pump (or alternator) had incorrect shaft length forcing misalignment?
Could tensioner be failing and allowing the belt to slip sideways?
