I had a similar experience a few years ago with my wife's car (a Honda Accord, not with a Saab). We got hit by a drunk driver that had prior convictions and was insured by a high-risk company. Getting the car fixed was a battle every step of the way...where the car was fixed, getting a decent rental car, and the whole thing about diminished value, etc. The car was basically rebuilt from the rear window back. As anyone would suspect it isn't quite the same, either in looks or the way it drives. We basically use it now for short trips around town.
First of all, take your wife to her doctor if she hasn't already gone. If ever in another serious wreck I will not refuse a check-up even if I think there is no injury. You just never know.
As another poster mentioned, each state has different thresholds for totalling the car. Push as hard as you can to get it totalled. If not successful, get it fixed where you want and push hard for the diminished value. Couple of things the high-risk insurance compnay tried on me: Offered a fast "let's settle it" deal before I had time to research diminished value, etc. Take your time to do your homework. And don't sign or admit anything about your wife's medical condition until you're totally satisfied. You'll probably be interviewed (and probably recorded) by the insurance company. The medical part will be your biggest leverage to use if things go south, so leave it open as long as you can. You can say that she has been seen by a doctor and due to her pregnancy will continue to be closely monitored. And once you're in the rental car, don't give it up until you are TOTALLY satisfied. The daily rate starts to add up on their account and begins to exert pressure if things drag on.
I'm not advocating any form of insurance fraud, but the other company will try to get out for the least amount possible. Just be sure you are treated fairly and compensated fairly for your loss.
