Your Coolant by-pass Valve connects to three hoses. Your description makes it sound as if one or more of the hose clamps were not re-installed properly when valve was replaced (if original clamps were reused) or may have loosened with time if worm-gear screw type clamps were installed with new valve.
If worm gear screw type clamps, tighten them. I had to use two long extensions and a small (8mm?) socket to reach and tighten clamps when I permanently eliminated my CBV last week.
