Excessive engine revving

An item of concern popped up a couple of times this past weekend...
One time, while idling at a light (tranny in drive), the engine started revving up to the point I had to really step on the brakes to keep the car from moving. It settled down by itself after a couple of seconds.
Second time, I was stopped outside a store (tranny in drive again) waiting for my wife to come out when the revving went high enough I had to shift into neutral and let it settle down. Almost was going to shut the engine off but it settled down as well.
Any thoughts out there?
