Has the fuel pump been replaced with the insert previously? Saw two complete failures and one partial failure of this repair method last year. On a OG9-3 and OG9-5, the line popped off the insert or pipe which gave a no start condition. The other was a NG900 2.3L that developed a P1128 long term fuel trim code. Usually for a post AMM air leak. Could find nothing, and replaced the purge valve. Ends up the previous repair shop had used regular fuel hose to make the union which over three years had softened up and split. Providing some fuel pressure but not quite enough. When starting, gave the same symptoms you describe.
