The purpose is to remove gum, carbon and the like from the intake and valves. The value depends on many things, most importantly, how much goo and carbon there is to remove.
I think it can be worthwhile and result in removal of foulants but doubt that one application can remove all if fouled badly.
Many will suck it into the intake manifold thru a vacuum line. It makes lots of white smoke, really a lot. Best if you have one person on the accelerator to keep the car running and one running the hose to suck it out of the can.
I have done the process of the intake before and not noticed any difference but I doubt that I have much contamination.
I now use seafoam in place of any other injector cleaner by pouring the can into the fuel tank.
