in general terms, I would expect that the filter is upstream of the fan, filtering only recirculated cabin air. It could be filtering the mix of outside/recirc air, this would better protect the cooling and heating coils. However, SAAB's reasoning could be that it protects against pollen and other allergens, and noxious odors. If the user switches to recirc in those conditions, the work expected of the filter would be greatly reduced.
A '98 with 69,000km could well be do for a new filter. A dirty filter will put additional strain on the fan motor, creating more noise. I would expect this to be a gradual noise increase. If you noticed a sudden increase, a bad motor is more likely. The bearings are probably not servicable. It seems unlikely that any debris could make its way to contacting the blade, and stay in contact with the blade, but I won't say that it's impossible.

On recirc, all return air to the fan is coming from the cabin. You will hear all of the intake noise of the fan, as well as the exhaust. With recirc turned off, you hear the exhaust noise only (unless it's not 100% OA, and some is recirculated, which is possible). The fans of all cars will be louder when in recirc., and they will probably discharge more air. It is easier for the fan to return air from the same space that it is supplying it to, than draw from the outside, and force the relief air out of the cabin.
