If an O2 sensor is bad enough to change the gas mileage in a significant way, it will set a code. That said, it is not a horribly bad idea to replace them, like say, 100K miles if you want to chase perfection.
Starting problems cold have nothing to do with O2 sensor since that sensor is not in charge at cold start but kicks in some 30 sec to a minute after start. Air mass meter, coolant/air temp sensor, manifold pressure sensor and vacuum leaks are more likely culprits.
Gas mileage is horrible in city driving and all depending how featherlight on the pedal you are at each stop sign.
My 9-5 gets 33 mpg at gentle freeway driving and 12 mpg going back and forth to work.
