Leaves in AC?

It sounds like some leaves or other debris have somehow entered the blower blower motor of the climate control system. The sound is really loud when the climate control is set to recirculate, and more or less goes away when it is not in recirculate mode. I have three questions:
1) If there is a filter, then how could the leaves have entered the system at all?
2) Why would the sound be more prevalent when the climate control is in recirculate mode?
3) How would you recommend eliminating this annoying problem?
