Extended warranties (which is all the certified program is) are like most other insurance products. Some people like the security they provide. Others realize that someone makes money selling them, which means you have a pretty good chance of coming out on the short end if you buy one.
The cars my wife drives lead a hard life. So we got a certified 9-5, and sure enough within a month, we noticed the infamous smoke on cold startup The dealer we bought from didn't bat an eye. They put a new turbo in and we had a nice Arc loaner for a couple days.
Don't know what a turbo replacement would cost, but my guess is we just made up a fair bit of the cost of the certified car.
Of course, you could say I should have been more careful when I bought the car and avoided one with a bad turbo, but that's another thing about insurance. Sometimes it protects you from bad decisions.
