I've had headlight fog up too but only when a rock or something else put a hole in them. The cause could be a leaking gasket between the lens and the rest of the unit, or possibly the drain hole is plugged, or it could just be the weather where ever you are. I am familiar with condensation problems with aircraft passenger windows. There are two possible causes for this, either a leaking gasket or excessive humidity in the passenger cabin. The gasket problem would be different with a headlight as a leak wouldn't cause pressurized air to flow through the headlight as it does with passenger windows. In the headlight case, the leak would allow water to get in somehow. As to the excessive humidity this would apply in much the same manor, when the lights are on the headlights get hot and some air is forced out as it expands, when you turn them off, they cool and draw in outside air. If it is very humid, this will draw in some moisture which will eventually condense on the lens if enough gets into the headlight.
As to what to do about it? Wait for summer usually works but the moisture can damage the reflector so it is best to do something about it sooner than that. What I for the rock hole was, take out the headlight, separate the lens from the rest of the unit and dry them both. My method was bring them into the house and leave them sitting on heat vents for a couple hours. I also sealed the hole with clear silicone sealant, that worked just fine for a couple of months until I replace both lenses with E-Codes (silicone has been working for 15 years on the wife's Fiat). While they're apart, inspect the rubber gasket for cuts, tears, etc. If you find any, you will need to replace them.
