The throttle has a mechanical input that moved a variable resistor. The ECU controls the actual throttle plate. It compares the postion of the mechanical input and the actual throttle plate and takes things from there. It can stroke the throttle plate a little faster for better throttle/boost response. If the ECU things that things are bad, it will trip a limp home solenoid to mehcanically couple the throttle and mechanical input. That makes it so that throttle cannot be openned all the way, so power if very lacking when that happens. As the ECU is in control, the ECU can move the throttle plate for idle speed air control and for cruise control. So the IAC valve and the separate cruise control unit are not needed any more.
If the TB gets deposits that make the throttle plate start to bind, this may lead to a limp home mode or damage. So cleaning the TB every couple of years would be a good idea. Early TB's had lots of failures it seems, but I get the impression that this is now infrequent. You do not need a spare TB in the trunk as it will not fail to a poing where you cannot drive.
