Could very well be because in the "old" days with the red DI cassette and the 9000 model the car would just run worse and worse and worse until it was so bad that even the driver thought it was something wrong and brought it in for repairs.
The break down would happen so slowly that the driver got used to it (just like worn plugs).
Actually, if you read ALL the black DI stories (who have the time for that?) you will pick out a voice here and there where they mention misfire or hesitation as a prelude to failure.
It could also be that many of the "sudden" failures has actually been showing signs early on but the driver had gotten used to it and didn't notice anything until the car died.
We are all so different in perception of how a car run, I notice all the hick ups my car makes but I can most of the time relate to what triggered them and don't worry about it.
