Which the dealer *should* handle. Some folks are more sensitive to brake pulsations than others, but if you can demonstrate it to your service advisor I would expect a "highline" dealer to handle a complaint like this.
Were the brakes redone? Were just the pads done and not the rotors? Dean's post is right on, you might fix this your self by bedding in the brakes better.
As for the wheel balance, are the tires new? Then it's a warranty issue with the tire installers. Even if not a decent dealer should address the issue at no charge if you've brought it to their attention relatively soon after the sale, like less than 30 days.
I'm amused that they lost the 110 point inspection form, it's a critical document for Certification. I find it hard to believe that it was truly lost. I suspect it's lost because they thought it might show some marginal brake or tire measurement issues.
If they give you any grief over the brake and wheel balance issues I'd be doubly suspicious enough to have an independent look at the brakes and tires.
You paid good money for the Certification. You wouldn't be outa' line asking for it in writing. The mechanic who performed the orignial inspection should be able to reconstuct the form from memory except for the brake and tire measurements. Most Certfied 01 9-5's need tires and brakes unless recently done, even with low miles.
