Fuses rarely do that, but never hurts to do a visual check.
Could be the speed controller module failing, typically they fail dead or sometimes fan will not shut off at all, even with key out. Could be a connector at the motor or controller, but that seems like a remote possibility.
The fan gets power (+12V) all of the time, not switched power, the speed controller acts on the motors ground lead. If the controller shorts out, the fan will run until the fuse if pulled or the battery is dead or disconnected. If the controller fails so that it will not sink any current from the motor, the motor will not run at all.
Any prior work under the dash or air box filter replacement? Anything that would have disturbed grounding points? Audio or cel phone work?
