I'd guess vacuum leak. Look even more carefully for hoses that have fallen off. Check every hose, and make sure you know where it starts and where it ends. Inspect the hose for cracks and splits, especially where it meets a fitting. This is true for the big air hoses and the little air hoses. Check the gasket on the underside of the oil filler. If that gasket is bad, you have a big air leak (air enters the pipe, gets into the crank, and gets pulled into the intake manifold). While the car is idling, move hoses around and see if the idle changes - if it does, inspect that hose carefully. Check the hose from the intake manifold to the fuel pressure regulator. Pull the hose off the intake and suck on the hose. If it tastes strongly of gasoline, the diaphram of the fuel pressure regulator may be leaking, and that can mess up the idle.
Also check the hose from the intake manifold to the distributor - the vacuum retard. Pull the hose off the intake manifold and suck on it. It should hold vacuum. Then pull the distributor cap off, and suck hard on the hose. You should see the base inside the distributor move a little, showing the vacuum retard is working. You get max vacuum with the throttle closed - i.e. - idle.
This is but a start, but it's rare to see a car that's been around for 10+ years that doesn't have at least one vacuum leak. If you find one, I'd keep looking.
