Coughs when she runs a fever

I have a ?89 900 S with a manual transmission and 170,000 miles. She runs fine during typical ?city driving?. Also runs fine for highway driving. But when I get off the highway, the car can?t seem to go back into the stop and go city-driving mode without getting what can only be described as the hic-ups. While trying to accelerate or decelerate at lower revs, the car can?t make rpm adjustments quick enough and acts starved for fuel (or air?) then almost stalls, then surges. It does this regularly after long highway runs, but not always. I can usually continue driving by revving it up and keeping the rps higher.
I?m thinking there is some type of vacuum/pressure problem in the fuel system once the car get upto normal operating temps. The hic-up problem seems to coincide with the car running warm. Also, the car idles high 1500 rpm when it?s warm. Why wouldn?t it idle at the normal 800 if it?s warm?
Recently replaced the thermostat. The temperature gauge indicates the car is operating at normal temperature on the highway.
Thank you for any suggestions.
And for extra credit????..why does my gear shift knob rotate clockwise? A full rotation coinciding with the lunar cycle?
