The mirror is held with some screws behind that triangular plastic piece on the door.

Here's the details.

The mirror housing is held to the mounting base (the part that attaches to the door) with spring tension so that the housing will give some if it's bumped. A T-shaped casting carries the spring load and can become distorted over time allowing a little slop to develop.

The Fix:

Remove mirror from the car: remove door panel, pry up the bottom of the triangular plastic piece and rotate to expose two screws.

Remove mirror glass. Push the bottom of mirror all the way into the housing so the teeth on the locking ring are visible. Use a small screwdriver through hole in bottom to rotate the toothed ring inward (or is it outward?).

Remove the motor assembly. Undo the 4 screws, undo the wire grommet and slide the wire out. If the T-shaped casting is badly deformed, the plug may not have sufficient clearance to be pulled all the way out. If so, just let the motor assembly dangle while you drill out the plastic studs.

Drill out the four plastic studs that hold the steel frame in place. Use a 3/8" bit or larger and just drill enough to go through the plastic. Don't go into the metal.

Pry the steel frame up from the outer end, then pull the frame outward a bit to free the tab on the inboard end of the frame from the mirror housing. Remove the steel frame from the mirror housing. You'll be fighting spring tension as you do this. Once the steel frame is clear of the housing, you can disconnect the springs so the mirror housing can be separated from the mounting base.

Drive out the split pin holding the T-shaped casting to the mounting base.

Using a vice (or hammer), flatten out the T-shaped casting. I'd be leery of using a pliers to twist/bend the casting because the nicks could promote cracking. This part is clearly not designed to handle the load applied to it.

Reassembly is the reverse of disassembly. I used four ½" #7 flat head screws to secure the steel frame to the mirror housing. The plastic studs on mine were hollow and didn't require drilling.
