The shifting issue could be a problem w/ the alignment: either the clamp where the shift rod connects to the mating rod coming out of the transmission, or the rubber damper gizmo on the shift rod has come apart. See the Bentley manual for the alignment procedure. You could also have a problem w/ the shift mechanism under the center console if something has broken or come loose.
If it grinds going into reverse, the problem is probably either that the clutch isn't disengaging fully (elongated hole in clutch pedal, bad master or slave cylinder, air in line, etc.) or the clutch shaft bearing is shot.
Popping out of reverse could be a sign of a damaged reverse gear, the fix is described on the Townsend Imports web site.
However, the collection of transmission problems could also be a sign of a bad pinion bearing, particularly if you've had a whine in 3/4/5 above about 30 mph. As the pinion bearing lets loose, slop develops in the works and shifting gets chancy because of accumulating damage elsewhere.
