You are experiencing a classic problem. The holder is not making tight contact with the plastic film on the back of the instrument panel. If a garden variety #194 5 watt bulb has been lighting your dash, it's only a matter of time before you will have melted a dimple in the hard plastic causing a loose fitting. You need a 3 watt bulb (looks identical to 5 watt).
Temporary fixes range from: whacking the top of the dash, bending out the contact on the bulb holder, spraying with WD-40. Permanent fix is to peel back the printed circuit and fill the dimple in the hard plastic with epoxy like JBweld and a toothpick (special tool). You can do it through the speaker hole, if you get a mirror so you can see what you are doing.
