It's usually an air leak of some sort.
Have you replaced any of these things:
vacuum hoses
rubber bushings that hold the vacuum nipples into the intake manifold and valve cover
rubber bushing that holds the large PCV hose to the duct between the AMM and turbo (I changed that one out and got rid of hesitation when the engine is cold)
and have you checked the turbo bypass valve to see how it holds vacuum.
Also, make a close inspection of all of the boots that connect the turbo ducting together. Pick 'em up, fold 'em, and see if there are any cracks in the rubber and make sure that they're well seated and clamped.
