If the cap is off, the cooling system is not pressurized and if the coolant temp is 80-90 C, evaporation naturally produces air bubbles (which is supposedly prevented if the system is pressurized.) If I understand correctly, under the condition you are testing, bubbles may not be from external. After you drive until thermo opens and the system gets pressurized sufficiently (half hour?), and without removing the cap, do you still see those bubbles? I hope you don't. And if this is the case, air may get in the system simply by loose clamps, seals, or cap packing, but not from combustion system.
