The shifter knob is held in place by friction. Put the transmission in neutral, and pull up hard. Don't look straight down at it while you're doing this, or you'll punch yourself in the face.
The bottom of the shift boot is held in place by a rubber "lip" which has been stretched around a metal edge ~2mm above the console. You can pull it off and on with your fingers. Leather shift boots are slightly more difficult to remove and replace without damage.
If you need to remove the entire shift column, unbolt the 3 bolts which retain the upper cage for the big plastic ball. When replacing, watch out for the small plastic tab which prevents the shift column from rotating.
