Limp-home has its own fault code, 12221. The AIC circuit fault (12222) could be dirty/loose connections to the AIC valve or to the throttle switch. OTOH, it coule be a bad AIC valve, too. However, that system is only active when idling, so that should be unrelated to your driving problem.
FPR doesn't have a code; since its not electronic, the ECU can't directly tell if its screwed up. But it can cause other codes like 12111. Pull the vacuum hose off the FPR, there should be no gas in there. I don't know if FPR can go bad without leaking gas into the vacuum line. I guess you need to measure the fuel pressure to find out.
12111 just means that, for some reason, the lambda system can't keep exhaust O2 within its acceptable limits. The ECU doesn't always know why... If you don't get 12223(lean) or 12224(rich) along with it, then you don't really know which it is.
The noise is very strange. You don't have a turbo, so the crankcase can't(?) get pressurized. If it were a vacuum leak, it shoud be stronger at idle when the vacuum is stronger.
Get a better idea about the noise. Narrow down the location. Have somebody try to rev the engine while you listen for the location of the noise. Describe the circumstances: can you make it louder, softer, etc...
