New Oxygen Sensor not outputting voltage

HI. Just installed new "universal" Oxygen Sensor in my '91 N/A. Am getting ECU error codes 12112 and 12111. Checked for signal output from sensor once engine warmed up and saw zero volts (actually about 30 mV). Checked for voltage at preheater connector and saw battery voltage there. Checked preheater coil resistance and got correct 4.7 ohms. Disconnected vacuum line to Fuel Pressure regulator and plugged the loose line--then the Oxygen Sensor output 0.75 volts due to very rich mixture. Thought maybe a vacuum leak somewhere leaning out mixture but could not find any leaky hoses/connections. Checked vacuum at idle--steady 20 inches Hg--seems normal. Checked for proper case ground at Oxygen Sensor--good solid ground (1 ohm to battery negative). I'm at a loss. Do these things sometimes not work when new? Finally, if I let the warm car sit for a few minutes, then restart, I do get a .5 to .6 volt signal out of the Oxygen Sensor after about 15 seconds, but then 30 seconds after that the sensor output goes to zero and stays there. Any advice greatly appreciated.
I'm sure I correctly soldered the leads from the "universal" sensor to the old harness. I'm an electrical engineer and have soldered plenty of wires in my day!
