The rotor itself has a shaft with a gear on the bottom. That gear connects with the cam. There is NO KEYING for that gear. So you can take the distributor out, spin the rotor, and it will go right back in. The engine will never start, and you'll be unable to set the timing with a light.

When you pull the cap off the rotor, you'll see a little mark on the body of the distributor. With cylinder #1 at TDC, the rotor should point approximately at that mark.

Before I pull the distributor, I turn the engine over so that the rotor points at that mark. I then pull the distributor. When the distributor goes back in, I line up the rotor with the mark as I slide it back into the engine. This assumes the engine has not be rotated! This will get you close enough to get the engine started and use a proper timing light.

In cases where the engine has been moved - say I've taken it apart, or had the timing chain off or the like - I make sure the engine is turned to cylinder #1 at TDC. Then I slide the distributor back in, and further verify that the rotor is pointing roughly to the cap location of the #1 spark plug wire.

Remember, the engine turns twice for every single rotation of the distributor, so it's easy to get screwed up. You want the distributor rotor to be pointing to the proper spark plug wire near TDC on the COMPRESSION stroke for that plug. You can be 180 degrees out, and have the plug fire on the exhaust stroke. Won't work very well.
