My experience has been:
If it's gear-related, then it occurs within a particular speed range. This usually means an out-of-balance or out-of-round condition for the tires, & you feel it when the oscillation gets strong enough to start the wheel bouncing. It can also happen at more than one speed, & that may mean more than one tire is out. Usually, bad front tires cause the steering wheel to shake more, while bad rear tires tend to cause more of a general vibration.
If it's RPM-related, you should feel it in every gear when you hit that RPM range.
Obviously, CV joints are only turning as fast as the wheels, but I believe it's the torque that creates vibration from these, & under acceleration they'd see the same torque at the same RPM in each gear.
Check the tires for any weird wear spots or uneven wear patterns on the tread. If so, balancing probably won't help much at that point; new tires & an alignment is the fix. If not, then as Andrew suggests, I'd have the wheels balanced 1st. If you have wider-than-stock rims/tires, have the shop split the weights evenly on both the inside & outside of the rim, or go to a place that does on-the-car balancing, which takes rotor imbalance into account. An alignment probably wouldn't hurt, either. Remember that doing both of these things is a LOT cheaper than replacing even 1 driver.
